Health and safety - accident reports and investigations



How we provide this service

Reporting accidents is supported by Community & Neighbourhood Services, Safety & Environment.

Investigation of accidents and injuries at work are undertaken by the Health and Safety section.


- If one of your employees has an accident,
- a member of the public is injured on your premises and is taken to hospital
- or someone is killed either as a result of an accident at work or because of the work activity on site

you will need to report the accident by completing a F2508 form.

This is legal requirement under The Reporting of Injuries Diseases and Dangerous Occurrences Regulations 1995 (RIDDOR)

What to do to get this service

You can download a F2508 form here. Once it has been completed it should be posted to:
